TWIST vs SkinPen vs DermaPen — what KW clinics use and why it matters
Every Kitchener-Waterloo clinic uses one of three microneedling devices. Here's an honest comparison of TWIST, SkinPen and DermaPen — written by someone who's trained on more than one.
If you’ve been researching microneedling in Kitchener, Waterloo or Cambridge, you’ve probably noticed every clinic uses a different device — and acts like theirs is the only legitimate option.
- Skin Vitality uses DermaPen.
- Sunshine Cosmetic Clinic / KW Medi Spa + The Cosmetic Surgery Clinic use SkinPen (“the first and only FDA-cleared microneedling device”).
- Silkentouch (us) uses TWIST for face and neck, and Procell for advanced cases.
All three devices microneedle. None of them are magic. But they’re not interchangeable, and “FDA-cleared” doesn’t mean “best for you.” Here’s the honest breakdown.

If a clinic is pushing a specific device without asking about your concern first — that’s a vending-machine clinic, not a personalised one.
How the three actually differ
SkinPen (Crown Aesthetics)
- The FDA-cleared marketing angle is real — SkinPen was the first to clear FDA in 2018.
- Straight-needle vertical action. Standard pattern.
- Strong brand. Aggressive sales support.
- Used by chain clinics in KW (Sunshine Cosmetic, The Cosmetic Surgery Clinic).
- Honest assessment: predictable, well-marketed, no major innovation in the past 5 years.
DermaPen (Equipmed)
- Also FDA-cleared (earlier, less marketed angle).
- Straight-needle vertical action. Similar mechanism to SkinPen.
- Used by Skin Vitality as their default.
- Honest assessment: solid device. Largely a marketing-equivalent to SkinPen with different branding.
TWIST
- Spiral-tip rotational action — different mechanism.
- Twisting motion creates micro-channels with less lateral tissue tearing.
- Result: more uniform collagen stimulation + faster recovery.
- Used by Silkentouch and a small number of other Ontario studios.
- Honest assessment: better tip technology, less brand recognition. If you don’t know what to ask for, you’ll be sold SkinPen.
What actually decides the outcome
Honest truth: the device matters less than the operator.
A skilled esthetician on DermaPen will get better results than an unskilled tech on TWIST. The factors that actually decide your outcome:
- Depth calibration per skin area — the operator chooses 0.5mm vs 1.5mm vs 2.5mm based on what they see. Wrong depth = no result or scarring.
- Number of passes — 3 passes vs 5 vs 7. Too few = no effect. Too many = scarring.
- Topical serum applied during — affects penetration and outcome.
- Aftercare protocol — the 72 hours after determine if you keep the result.
If a clinic markets the device instead of the practitioner, ask “who runs my session, and how long have they been on this device?” If the answer is “one of our technicians” or “depends who’s available”, run.
Why we picked TWIST
When I built Silkentouch, I trialed all three devices. Honest reasons I picked TWIST:
- Less tissue tearing = faster healing. My clients drive from Kitchener, Waterloo, Guelph — they can’t take 5 days off.
- More even collagen stimulation = more predictable results. The rotational motion is harder to do wrong.
- Lower cost per cartridge = lower price I can charge you. Silkentouch’s TWIST is CA$250/session vs DermaPen sessions at chain clinics typically CA$350-450.
I also bought a Procell unit for advanced cases. When TWIST isn’t enough — deep acne scarring, stretch marks, advanced laxity — Procell’s stem-cell-derived growth factors deliver something neither SkinPen nor DermaPen offers.
Why I didn’t pick SkinPen or DermaPen
- Straight-needle motion tears more tissue per session. Longer recovery for clients.
- Brand markup. SkinPen pricing factors in their marketing spend. Clients pay for the ads.
- Less flexibility. Both are good devices, but TWIST + Procell combined gives me more range for different concerns than either single device alone.
This isn’t a knock on Skin Vitality or Sunshine Cosmetic — both run reputable practices. It’s just that I wanted to optimise differently.
What to ask before booking microneedling anywhere
- “Who personally runs my session?” If it’s not the same person every time, calibration history doesn’t carry over.
- “What depth setting will you use, and why?” Vague answers (“we’ll customise it”) = no real plan.
- “Show me before/afters from clients with similar skin to mine.” Generic stock photos = no actual portfolio.
- “What’s your aftercare protocol?” If it’s a one-pager handed at checkout, results suffer. (We send a personal text on day 1.)
- “What does this cost per session AND per package?” Hidden pricing = upsell incoming.
